Your initial appointment will consist of an evaluation and consultation explaining your diagnosis and treatment options. Dr. Cullum will review your specific concerns and help you design the right treatment plan for you. Occasionally, with advanced preparation some procedures can be done the same day as the consultation.
In our experience, patients do much better with treatment when they have a good understanding of what to expect at their appointment. It is often helpful or necessary to use certain medications the night before or morning of your treatment. If you have a complex medical history or treatment plan, it may be necessary to consult with your physician or dentist prior to treatment. A pre-operative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Instructions for IV anesthesia will be given to you at consultation in preparation for your surgery appointment.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any X-rays if applicable.
- A list of medications you are presently taking.
- If you have medical or dental insurance, bring your information and the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or guardian must accompany patients under 18.
Please let us know if you have:
- A medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, heart murmur, rheumatic fever, cancer treatment with chemotherapy or radiation, etc.)
- If you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
- If you are on any herbal or home remedies, special diet, nutritional supplements or diet suppressants.
- If you have had difficulties with any anesthesia in the past.
- If you have any allergies to medications, antibiotics, latex, etc.
- If you are now, or could be pregnant.
- If you have any special condition, needs or concerns.
X-Ray Images
If your dentist or physician has taken x-ray images, you may request that they forward them to our office. If there is not enough time until your appointment, please pick them up and bring them with you to our office. If you have not yet had x-rays made, we will complete the necessary images at our office. Sometimes additional views are necessary for complete diagnosis or for any interval changes.
