Initial Office Visit

At your initial visit, we will develop your treatment plan, an estimate of related fees, and work with you to arrange payment plans. We will examine your teeth and gums with focus on areas of concern, perform a cancer screening, and take your blood pressure. Dr. Cullum will assess and discuss your personalized treatment options. If you have a complex medical history, it may be necessary to consult with your physician or dentist prior to your office visit.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any X-rays if applicable

  • A list of medications you are presently taking

  • If you have medical or dental insurance, bring your information and the necessary completed forms.

Taking these steps prior to your consultation will save time and allow us to help you proves any claims.

IMPORTANT: A parent or guardian must accompany patients under 18.

X-Ray Imaging

If your dentist or physician has taken x-ray images, you may request that they forward them to our office, or you can bring them with you to the consultation. If you have not yet had X-Rays taken, we will complete the necessary X-Rays at our office. Sometimes additional images are necessary for complete diagnosis.

CT Scans

Our practice utilizes the latest in 3D diagnostic imaging technology with an I-Cat Flex CT Scanner. The 3D imaging allows us to see potential problems and provide you with the best comprehensive treatment options.
